You will need a nominated invigilator to invigilate your examination. This might be a Head of Service, Deputy Head, GP, Senior Nurse, Medic, Supervisor or other senior member of staff. They will need to complete and sign this invigilator consent form each time an examination is performed, which confirms that they ensure no other material was used in the examination and that the examination was completed by the named candidate.
